Like face-lifting, eye-lifting is an anti-gravity operation. With time and age, the upper eyelids become hooded, making the eyes appear smaller and creating a 'tired' appearance. At the same time, fat drops from under the eyeballs to the lower eyelids, causing puffiness and dark shadows under the eyes.
The upper eyelids can be dramatically improved by removing the excess skin and muscle and creating new eyelid creases. Excess fat is removed at the same time. A very fine, almost invisible scar is created in the eyelid crease, which extends into the wrinkle lines at the sides of the eye.
To remove puffiness under the lower eyelids, fat must be removed, and occasionally excess skin and muscle are trimmed as well. The traditional approach was to make a cut just below the eyelashes, then cut through the muscle to reach the fat pads. This approach often caused pulling on the lower eyelid, altering the shape of the eyes and causing constant redness. To counteract these problems, the fat is now removed by making the cut on the inside of the eyelids. This means that the fat is more accessible, there is no external scar and the lower lid is not pulled down. Although excess skin cannot be removed with this approach, it is rarely necessary to do so because the skin red rapes itself back into its original contour once the fat pads have been removed. Eye surgery can now be performed under twilight sedation as a day patient procedure. In the last five years eye-lifting has been greatly improved with the use of lasers. The procedure can now be performed with almost no bleeding or bruising, so that people can return to work within four or five days. Pain and swelling are also reduced when lasers are used.
